The larger patch is necessary for those players who bought the original Sacred, and it contains the free Plus update. The smaller patch is for players who have either bought Sacred Plus a version of Sacred which includes the first Plus update, v1.
To make matters more confusing, some versions of Plus were sold in Original Sacred boxes, so the best way to determine which version you have is to check the version number.
